This form should be completed by any Northwestern International student who needs to report any changes to their SEVIS record during their approved Optional Practical Training (OPT) period. It is a requirement that any student who applies for OPT continues to maintain communication with the International Student Adviser and inform them of any changes within ten (10) days in order to remain in good status.
